  • The request must specify the right you wish to exercise and, where applicable, the arguments or documents supporting the request;
  • If the request is made by a representative on your behalf, the request must include not only the above information, but also the name, address and contact details of your representative.

Ways to verify your identity:

  • When applying directly to the Company's office: by presenting an identity document to an employee of the Company;
  • In the case of a postal application: the application must be accompanied by a notarised copy of the identity document or a copy of this document certified in accordance with other legal procedures;
  • By email request. Requests submitted in this way must be signed with a qualified electronic signature or in an electronic format that ensures the integrity and irreplaceability of the text (unless you revoke your consent to the processing of personal data in accordance with this Policy).

Signing the application with a qualified electronic signature allows the Company to properly identify you and ensure that the information you request or information about the execution of your requested actions (refusal to perform) is disclosed only to the person to whom it is addressed, i. y.
